| HOW TO REGISTER
1. ON-LINE: Don’t want to stand in line? Want to pay by credit card? Log on to www.snb.ca/fdsa to register for 2010 . This is a secure site created by Service NB. To register online, you must use a credit card, have an email address and personally register yourself. Online registration will run from Monday, March 8th until Saturday, May 1st. Please ensure you receive a REFERENCE number upon completion of registration; if you do not receive one, the registration process was NOT completed. Keep your receipt for your tax credit.
2. IN-PERSON: At any of the following locations and time (cash or cheque ONLY):
a. FDSA Office: Beginning March 16th from 11:00am - 2:00pm (Monday to Friday)

Friday, April 2nd - GOOD FRIDAY
Monday, April 5th - EASTER MONDAY
b. FDSA Office:
1) Sat. March 20th: 10:00 am - 1:00 pm
2) Sat. May 1st: 10:00 am - 1:00 pm
(LAST DAY for 2010 registration ).
3. Division I and Premiership:
a. All Premiership players must be registered NO LATER than Saturday, March 20th @ 1:00pm.
b. All Division I players must be registered NO LATER than Friday, April 16th @ 2:00pm.
c. No registration will be held on the first day of Premiership or Division I tryouts.
d. Individuals MUST be registered prior to participating in tryouts.
4. PLEASE NOTE:
a. REGISTER EARLY. Registrations submitted AFTER the deadlines outlined will be subject to a $25 late fee and may be placed on a first-come, first-serve wait list.
b. Registration DEADLINE: 1:00pm on Saturday, May 1st.
c. FDSA cannot accept faxed registrations. If you plan to register on line, you must personally register yourself; office staff cannot do this for you.
d. Premiership Players: Tryouts will run March 30th - April 24th.
e. Division I Players: Try-outs will run April 26th - May 22nd.
f. Family Rate: Families with 3 or more children in the FDSA program will receive a $15 discount for each child (must register all 3 or more children at the same time to receive this discount).
g. Each child (except Premiership & Division I players) will receive a soccer jersey to keep at end of season.
h. NSF Fee: A fee of $25 will be charged.
i. School facilities are subject to change at the last minute due to the priorities given to the district.
FEDERAL TAX CREDIT
Starting in 2007, the Government of Canada proposed to allow a non-refundable tax credit on eligible amounts of up to $500 paid by parents to register a child in an eligible program of physical activity.

GENERAL REFUND POLICY: Policy 14 section

If a child withdraws from the summer program prior
to July 1st, 50% of the FDSA portion of the registration
fees paid for that child will be refunded (the amounts
are: Micro - $31; Mini - $42.13; Youth & Senior Women
- $47.63). If a child withdraws after July 1st, no
refund will be issued.
PREMIERSHIP & DIVISION I TEAM FEE REFUNDS:

If a child has tried out for a FDSA Division I
or Premiership team and that child is not selected
for the team, the FDSA will refund all fees with the
exception of the tryout fees (Premiership = $50; Division I = $25 ).

Notwithstanding section 6.1.1 above, if the FDSA
decides, before the end of tryouts, that there are
insufficient players to form a Premiership or a Division
I team, then the total fees paid will be refunded.

NOTE: Refunds will NOT be issued until both Premiership & Division I try-outs are completed and team rosters have been finalized.
